Clinical Definition
Obesity is linked to daytime fatigue through several mechanisms: obstructive sleep apnoea and fragmented sleep architecture, raised circulating inflammatory cytokines (IL-6, TNF-alpha, CRP), reduced cardiorespiratory fitness relative to body mass, and increased work of breathing and ambulation. Fatigue may also reflect comorbid hypothyroidism, iron deficiency anaemia, poorly controlled type 2 diabetes or depression. Assessment should screen for sleep-disordered breathing, thyroid function, haemoglobin and glycaemic status before symptoms are attributed to weight alone.
